Once you blow up the picture some so you can see the details of the two separate steam engines, it becomes a bit clearer. TL;DR: double head on crash.

The two engines collided facing in opposite directions. It is unclear whether the unburdened engine was moving, but it appears it did not have it's tender hooked up, which leads me to believe it may have been simply parked in the wrong place at the wrong time. Anyhow, The foreground engine, which was not pulling anything, did a wheelstand, pivoting around it's rear drive wheel, as it tried to climb over the top of the background engine. Even if the unburdened steam engine was not moving, simple inertia of the two objects could have done this. The pivoting ended as the back end of the boiler hit the railroad tracks, crushing the crew compartment, called a cab. At this point the other engine and the train behind it used the last of its forward inertia to climb up the belly of it's victim.

That is what I think the story is here.

Speaking of balls and steam engines, "running balls out" refers to running a governed (speed limited) engine (originally just steam engines) at maximum speed, thus throwing the governor balls as far out as possible! Unfortunately this is the best gifs I could find of the principle are these two.

Sorry, should clarify: Vancouver and the west side not Canada. We've been civilized and modern over here in Upper Canada for some time. ;)

I still think this is a boudoir shot of Thomas the tank engine's parents...
